Registration

We will attach the 2026 forms for the Super Peewee season. Parent consent form, physical form, volunteer application and affidavit.

The affidavit form must be filled out and singed by the CEO, Executive Director or President of Organization and the name of the Association as highlighted on the form. The affidavit needs to be notarized by a notary. You only need to fill out one affidavit for your park. By signing the affidavit and having it notarized you are acknowledging that your volunteer Super Peewee coaches, team parents and any other volunteers background were checked, and they have passed the background. Return all volunteer applications with a copy of their ID with the affidavit at our first coaches certification or before. Certification date and location will be sent at a later date.
